My D.TUBE account is broken :(

I think my d.tube account is broken and I have no idea why. I've uploaded 3 videos to d.tube and everything works accordingly, but I tried to upload my fourth video, but I keep getting the same error. "Error: Please upload a video before submitting".

As seen on the screen shot, the video has been uploaded and a snap pic has been been also uploaded, all other fields have also been added. Anyone else encounter this problem? If so, please let me know on how to resolve this issue. Also I can't add comment to other user's video as well. It's like my entire d.tube account has been bork!
