The application of cognitive behavioural therapy for fear is an essential component of anxiety management. As part of the treatment, the patient will be asked to participate in activities that are outside of their comfort zone. Although it is ultimately up to the patient to decide what they will go through, the therapist can provide encouragement and give suggestions to make the process go more smoothly. The therapist may also assist the patient in partitioning the total amount of exposure into more manageable chunks.

The initial phase of the process involves the psychologist assisting the client in constructing a hierarchy of exposure to the client's feared things, activities, and situations. After that, the patient is made to experience these things until their fear rating reaches at least 4 out of 10. This exposure is continued until the client indicates that there has been a discernible drop in the level of fear.
