I recently posted an article on this very topic - the ventilation aspect is so key because it's the incomplete combustion of fossil fuels that releases CO. Burning fossil fuels in sufficiently aerobic environments (i.e. properly) will not release dangerous CO levels, which is where the importance of ventilation is derived.
