Case Study: Patellofemoral Pain Syndrome (PFPS)
A 38 year old female office workerr presente with pain in the front of her knee especialy while climbing stair squating or siting for long period. She describes discomforts during prolonge siting and paine while going downstairrs. The pain had been present for the last 3 month and worrsened with the activity.
Causes
- Overuse or repetitive stress on the knee joint prolonged sitting stair climbing
- Weaknes of quadriceeps muscles especialy the vastuos medialis obliqus (VMO).
- Malalignments of patella due to the muscle imballance.
- Poor biomechannics or flat foot increasing stres on the patellofemoral joints.
Symptoms
Dull aching paine around or behind the kneecaap (retropatellar pain).
Pain increased with stairs climbings runnings or sitting with bent kneees.
Swelling or grinding sound (crepitus) around the patella.
Discomfort during prolong sittings known as the theatere sign.
Diagnostic Test McConnells Test
To confirm patellofemoral joint dysfunction I perform the McConnells Test.
Procedure:
- The patient sit on a chair with the leggs hanging freely.
- The therapist place one haand just above the ankle and flexees the knee to about 120°.
- Resistance is applied as the patient tries to extend the knee.
- The same steps are repeated at 90° 60° 30° and full extension.
- If retropatellar pain appear at any of these angle the test is repeated in that same position while the therapist preses the patella meddially.
- The test is positive if pain appears during resisteed extension but is relieved when the patella is displaced mediallly.
Interpretation:
A positive McConnells Test indicates patellofemoral pain syndrome due to the maltracking or laterall displacements of the pateella.
Test Performance

Physiotherapy Management Protocol
Pain Reduction Phase
Cold Therapy (Ice Pack):
10–15 minutes to reduce inflammation.
Patellar Taping:
McConnell taping technique to correct patellar alignment.
Electrotherrapy:
Use of ultrasounds or TENS for the pain reliefs.
Strengthening Phase
1:Isometric Quadriceps Contractions
2:Tighten thigh muscles for 5–10 seconds × 10 reps.
3:Straight Leg Raises (SLR)
4:Improves quadriceps strength without patellar stress.
5:Mini Squats
6:Within pain-free range.
7:Clamshell Exercise
8:Strengthens hip abductors to stabilize patella tracking.
Stretching & Functional Phase
Hamstring & IT Band Stretching- Reduces lateral tension on patella.
Step-ups and Step downs For functional knee control.
Balance and Propprioception training Enhances knee Stability.
Exppected Outcomes
After 4–6 weeks of consistentts physiotherapy:
1-Pain relief and improved patellar alignment
2-Strengthened quadriceps and hip muscles
3-Better knee control during functional movements
4-Return to daily and sports activities without pain

But failed in task 3 as I asked users to perform exercises, and I gave the maximum grades on the basis of that task, but you didn't perform the exercises. Secondly, your posts look AI-written or AI for rephrasing content. It shows 15% AI written in the last paragraphs as you didn't even try to change the lines or their sequence. Giving maximum marks on the basis of task 3 was the main part of the challenge, as most of the users didn't participate in such tasks. I am really disappointed by this act, and unfortunately, it's an invalid participation.
