How are all my friends, I hope everything is as expected by the grace of Almighty Allah. Now back to the Steemit Learning Challenge Season 21 Week 6.
Stemit learning challenge competition for us on behalf of the Steem Physio & Rehab community, and This week's Challenge Organizer is @ashkhan on behalf of this community . I participated again in the six weeks of season twenty one and will try to give proper answers to all the questions of the competition.

What's disc herniation? Write in your own words after gating knowledge from the lesson post.

The spine is a stack of bones known as vertebrae, and these bones include small rubbery discs that act as a cushion. They have a soft jelly similar to the center or nucleus that is enclosed by a hard rubbery exterior. Sometimes this outer part of these rubbery discs can be torn and slip out soft inside This result is a herniated disc, also known as a slipped disc or ruptured disc.

This disc injury can irritate the surrounding nerves causing pain, numbness, or weakness in an arm or leg. Many people with slipped discs never experience symptoms, and surgery is rarely needed to resolve the issue. Still, there are a variety of treatments available to help, including those who suffer from pain and discomfort from herniated discs.


Cause and symptoms :


Most cases are caused by the wear and tear of a slipped disc. As you age some discs known as disc degeneration, your discs become less flexible and more prone to tears and ruptures. Most people cannot identify the cause of herniated discs. It can lift a heavy object using your back muscles instead of your leg and thigh muscles or from awkward twisting and bending. That said, there are other factors beyond your age that can increase your risk of disk slipping. Excess weight increases the pressure on the disc in the lower part of your back. Some people may be genetically prone to rupture of the disc. Working physically demanding 'and smoking can reduce oxygen supply to your disk, causing it to deteriorate more quickly .50% of people over the age of 45 to 50 suffer from the disease.


There are generally three types of disc herniation on the Level of the Region involved .


Cervical Disc Herniation


The Cervical spinal cord usually has seven vertebrae C1-C7 and C1-C8 8 pairs of cervical nerves from the cervical neck region.

Between every two bones there is a soft jelly-like part called the cervical discs.The two parts of this disc are an annulus f fibrous, it is outside the disk that is the nucleus pulpous that is on the inner side of the disc.If for some reason the strain falls or is injured, if torn, the annulus fibrouss is torn.


Symptom:


Neck pain, any work i.e. if the neck is turned to one side or if you work down, the neck hurts. This pain slowly creeps up to both hands.Many times the hands become paralyzed and if you take something in your hand, it falls or you can't put pressure on your hand or take anything heavy.
The entire hand and fingers are numbness, stiffness and tingling, and if the disease continues to increase day by day, the strength of the hand tends to decrease .

Cause


Sometimes it can be a C5 C6 disc bulge or a c4 c5 disc bulge may be another part of the cervical spine which in your neck,Disc bulges in the neck can cause tremendous amount of pain in the neck limit mobility,discomfort,agony, and even arm symptoms like numbness and tingling.


Thoracic Disc Herniation


The spinal cord & nerves root -Thoracic spine.
There are 12 vertebrae in the thoracic region T1-T12.the spinal canal in the thoracic region is relatively small.The spinal cord could be easy compressed or insured in this areas.Upeer back pain in the thoracic region can occur due to thoracic disc herniation.

Thoracic Disc Herniation Disease tends to occur due to aging, degenerative disc disease, and injury or trauma, usually as age increases.



Lumbar Disc Herniation


The lumbar spine is located down the back lumbar disc composed of five bones L1 L2 L3 L4 L5 5.
The lumbar spine compresses the nurve by increasing the discs backward then it is called the Lumber Disc Herniation.


Cause

This problem causes severe back pain along with severe leg pain.Suddenly it seems that the legs seem to be paralyzed along with numbness and tingling i.e. what is the pain like needle swelling. People find it hard to walk when pain is ever-shaped, sometimes urinating stools can stop.


Cause of disc Herniation

A Herniated disc is a medical condition that affect the spine. According accordingly 7 bones in the star views find 12 bones in the spine and five bones in the lumber spin . That disc helps to protect the bones by minimizing the impact of movements on the spinal column. A herniated disc can occur in any part of the spinal column ,they are more common in the lumbar spine just above th hips. A herniated disc can compressed nearby Nurve resulting in pain, numbness or weakness in arm and leg.

  • Degeneration can often be the result of a gradual , Age related wear turned the degenerative disc disease.
  • As people age, the intervertebral discloses its water content as well as the ability to cushion the vertebrae.
  • This reduction in fluid make the days less flexible and more prone to ruptureing even with a little twist or bend.
  • In some cases discarnation may offer in young in healthy people as a result of injury or tear from lifting large heavy objects.
  • This injuring may occure after twisting while lifting a heavy item or using the back muscles instead of the leg muscles while leaving heavy object.

Certain Factors that may increases the risk of a herniated disc may include:

  • Excess body weight
  • Genetics
  • An occupation that is physically demanding,
  • Sedentary Lifestyle.


A Herniated disc Symptoms


  • Pain in the neck shoulders or arm, numbness or weakness in the neck shoulders or arms.Numbness or weakness,And stiffness in the legs,
  • Loss of Bowel control,
  • A Difficulty with bladder control,
  • Pain or numbness that travels from the back to the front of the chest or upper abdomen.
  • President or continuous back pain that is worsened by movement sneezing ,coughing or standing for long periods of time.
  • Unexplained muscles weakness in the leg, Difficulty working.

Diagnosis


Diagnosis is made best on the history symptoms and physical examination.
Physical examination may aim to check reflexes, walking ability ,muscles strength the range of motion, sensitivity to touch and the possibilities of tender regions in the back. Is any other medical condition is suspected or the doctor needs to the location of the herniated disc and see which nurve is affected.

Doctor May order,x-ray, MRI imaging, CT scan or Mylogram.


Treatment


  • Medication,- Dr. suggest -Ibuprofn,Naproxen,
    If Medicine does not improve, the doctor prescribe codeine or oxycodone- acetaminophen.

*Therapy -If the pain does not resolve with in few week physical therapist may show patients position and exercise designed to help relieve the pain.

  • Surgery

Special test:


Spine x-ray - x-ray to see change in the spinal cord of conflict and xray on neck for neck pain and neck stiffness.

  • More serious Neck pain and stuffness or infection fracture MRI (magnetic regeneration imaging).
  • Milgram test if the patient feels pain in the spine to hold the leg up in 30-45 seconds.

Special test of cervical spine-


Cervical distraction test,Cervical flexion and rotation test.
Spurling test- eds are candidate sparling tests in cases of acute radiculopathy diagnosis.

  • The best test for diagnosing and observing Thoracic Disc Herniation is MRI.

Lumbar Disc Herniation:

Straight leg raising test,Sitting Straight leg Raising test
Lumbar flexion test,Passive lumbar extension test
Femoral nerve stretch test,
Cross SLR or Well leg Raising test +ve , Active SLR test
Passive lumbar extension .



How would you diagnose a disc herniation? Any clinical investment or assessment tests?

The problems I had--

Neck pain Any action i.e. if the neck is turned to one side or bent, the neck hurts. This pain slowly creeps up to both hands.Many times the hand becomes paralyzed and falls down with something in the hand or pressure can be given in the hand, no pressure can be given on the hand or anything heavy can be taken.

Whole hands and fingers jingle, stiffness and tingling and hand strength tends to decrease if the disease continues to grow day by day.

Moreover, sitting continuously for a long time starts back pain. Can't sit for long then it hurts a lot. Back pain slowly comes to the legs, then walking hurts the legs.
When the doctor stopped moving because of my problems, me some prescriptions with some medicine
When it comes to the doctor's physical medicine (PMR) with my problems, give me prescribes some medicine, Physical exercise and physiotherapy.

I took some physical tests X-ray and MRI. So I did the MRI report show.I and my husband have had problems so.So I did my husband's MRI report show.

William Flexion (knee to chest stretch)


This exercise is a very important exercise for lower back pain William Flexation. The first trick is to hold one leg up to the chest and hold both hands and hold it for up to 10 seconds.You have to lower your legs and relax again and hold this exercise on the other leg.

Stretches muscles and reduces pain. The advantage of this exercise is to open the intervertebral foramen and extend the ligamentous structure.

This abdominal exercise activates more of the abdomen and hamstring muscles as well as the lower muscles of the hip flexation and helps reduce pain.



Sciatic nerve glide


The main purpose of this exercise is to stretch the nerves. Pain on the nerves especially pain on the nerves helps relieve any inflammation and stress. It increases nerve motility. Stuck by tissue around the nerve near the joint, so doing this exercise activates it. This exercise has many advantages in the movement of peripheral nerves in the body.
Sciatic Acute back pain that extends to the feet.Pain is caused by nerve compression.

Sciatic nerve glide helps reduce acute pain. And improves speed and improves hip speed range.

Heating pad pain, soft tissue mobilization to reduce pain a lot.So five to ten minutes of heating pad will need to be applied before doing this exercise.

First, straighten your legs and fold them, then hold one leg high and hold it under your knees with both hands, then lower the leg once, so you have to exercise two legs five times



Hip bridging exercises


Hip pain can occur due to severe or severe injury or sitting for a long time or not.This exercise is done to strengthen the more back muscles, glutes, and root muscles of the mind. lumbar Reason Muscles Discs Herniation causes the muscles to weaken.So doing this exercise is essential to strengthen these muscles.

Avoiding the exercise if you feel pain while doing this exercise in its early stages is recommended.

In the initial state of starting, lie upright in a flat place, then fold both legs up to the knees, pull both hands close to the chest, and lightly raise the waist with waist pressure and hold it for five seconds, then leave the entire waist to bed.I had done this exercise five times.

Exercise William Flexation I tried a few times at first I had a hard time pulling my legs because it felt like I couldn't breathe to hold my legs close to my chest.
Since I have hand and hand pain and backpan, I did this exercise five times for 5 seconds at first. Then I tried doing this exercise three times a day. The pain in my hand has reduced a lot and the back pain that I had from sitting continuously now seems like I'm feeling a lot less now.


I didn't apply the heating pad first before doing the Sciatic nerve glide so every time I went to do the exercise one to two it turned out my legs were hurting. So I followed your method first I applied the heating pat for five to ten minutes on both legs then I did this exercise five times on both legs and once a day in the initial stage.

Then I tried to do this exercise five times a day twice a day so I found that I felt a lot less pain in the back when I sat down and a little less pain in my legs while walking.
What to do really increases nerve creep and pain reduces nerve pain.


  • Hip bridging exercises

Since I have severe back pain, I tried doing this exercise one to two times in the early stages. Then I did this exercise once a day when I saw that my pain felt less after doing the exercise.
So the above three exercises I've exercised every day make my back pain feel much less.
This exercise is a very useful exercise that greatly reduces back pain by increasing nerve mobility.


In fact, seeing the physical therapy exercises that you have given us to do for us in any of your learning challenges, I have recovered from physical pain at home by exercising.I think if you can do these exercises at least twice a day, we can be free from disc herniation disease.
So the above three exercises I've exercised every day make my back pain feel much less.

The primary treatment for the release of disc herniation diseasees is exercise. So I thank you and your community so much for uploading beautiful videos for us that we have been able to do many physical exercises and we, especially I, have benefited a lot.

Task 2 (2.6/3)
In the second question, you tell us about how you have to assess a patient by doing physical examination and history taking, performing investigations and special tests. But I asked about special tests in 2nd question and you added them in 1st one. Secondly you have share too much details then needed please share briefly about the asked questions. It would be better if you add name and explanation of one special test of each region to assess the disc herniation. Great.

Task 3 (3.7/4)
You try the hip bridging, William Flexion and sciatic nerve glides exercises . You did the hip bridging, William Flexion correctly. But in sciatic nerve glides you have dosri flex your feet while performing this. Always remember to apply heat pack before performing exercises to relax the muscles and reduce stiffness. I appreciate your efforts.
