Enfermería es el corazón dentro de la comunidad ya que se encarga del bienestar individual de cada persona, al estar en el área ambulatoria, se debe prestar servicio a todas las personas que habitan en ella independientemente de su condición.

Nursing is the heart of the community since it is responsible for the individual well-being of each person, being in the outpatient area, service must be provided to all the people who live in it regardless of their condition.

En esta oportunidad realizamos visitas domiciliarias dentro de la comunidad de Barrio Unión que se encuentra ubicada en la ciudad de Barquisimeto en el estado Lara en Venezuela, una visita domiciliaria consta de; como su nombre lo indica... Realizar una visita a un respectivo hogar, la intención de ésta visita es el interrogar a cada uno de los miembros dentro de la familia con el fin de obtener información relacionada al estado de salud.

This time we make home visits within the Barrio Unión community that is located in the city of Barquisimeto in Lara state in Venezuela, a home visit consists of; as its name implies ... To make a visit to a respective home, the intention of this visit is to interrogate each of the members within the family in order to obtain information related to the state of health.

¿Qué se pregunta en una visita domiciliaria?
Al momento de llegar a una casa primero debemos presentarnos como el personal de salud que se encuentra ubicado en el ambulatorio de la comunidad, luego de ser recibidos en el hogar con el permiso del familiar procederemos a realizar preguntas referentes a su estado de salud... Estas preguntas son variadas y se pueden extender si la persona presenta algún problema patológico o factores que lo desencadenen.
What is asked in a home visit?
When arriving at a house, we must first introduce ourselves as the health personnel who is located in the community outpatient clinic, after being received at home with the permission of the family member, we will proceed to ask questions regarding their state of health. These questions are varied and can be extended if the person presents any pathological problem or factors that trigger it.
Entre las preguntas principales está; datos personales como sus nombres y apellidos, edad, sexo, estado civil, fecha y lugar de nacimiento, antecedentes personales y antecedentes patológicos.

Estas preguntas se realizarán a cada uno de los miembros de la familia. Si alguno presenta antecedentes patológicos se procederá a abrir un expediente de SOAPE, lo cual significa que se le abrirá una historia dentro del área ambulatoria para así llevar un control sobre su patología.

Among the main questions is; personal data such as their names and surnames, age, sex, marital status, date and place of birth, personal history and pathological history.

These questions will be asked of each of the family members. If anyone has a pathological history, a SOAPE file will be opened, which means that a history will be opened within the outpatient area in order to control their pathology.


Con el expediente SOAPE y su historia clínica de enfermería se puede lograr llevar una morbilidad a nivel regional y estatal dentro del país.

With the SOAPE file and your nursing clinical history, you can manage a morbidity at the regional and state level within the country.

Gracias a esto, además de llevar un registro de morbilidad se debe orientar al paciente con respecto a su patología y sobre los riesgos que puede padecer si no cuida su salud, también se le otorgan los medicamentos necesarios si éste los requiere.

Thanks to this, in addition to keeping a morbidity record, the patient must be guided with respect to his pathology and about the risks that he may suffer if he does not take care of his health, he is also given the necessary medications if he requires them.



En esta ocasión visitamos a un paciente masculino de 89 años de edad quien se encuentra en reposo dentro de su hogar. Al momento de ingresar al hogar nos recibieron de una manera muy cálida respondiendo así al interrogatorio.


Al momento de entrevistar al paciente quien tenía por diagnóstico médico fractura en cabeza femoral derecha, nos indicó la forma en la que se ocasionó la lesión, el paciente relata que era un día normal como cualquier otro día... Él debía realizar los deberes diarios de su hogar por lo tanto entre esas actividades requería hacer el aseo, nos cuenta que al momento de sacar la basura que se encontraba en el segundo piso de su casa, se resbaló y el peso de su cuerpo calló sobre su pierna derecha ocasionando de esta manera la fractura en la cabeza femoral derecha, además de la fractura también presentaba lesiones en varias zonas de la piel como su rostro, espalda y rodillas.

On this occasion, we visited an 89-year-old male patient who is at rest in his home. When we entered the home, they received us in a very warm way, thus responding to the questioning.


When interviewing the patient who had a right femoral head fracture as a medical diagnosis, he told us the way in which the injury was caused, the patient reports that it was a normal day like any other day... He had to perform his daily duties of his home, therefore, among those activities, he required cleaning, he tells us that when taking out the garbage that was on the second floor of his house, he slipped and the weight of his body fell on his right leg, causing this Likewise, the fracture in the right femoral head, in addition to the fracture, he also presented injuries in various areas of the skin such as his face, back and knees.

Al momento de entrevistar a su familia y orientar sobre que el paciente debe ir a un centro médico para ser hospitalizado ya que requiere intervención quirúrgica la familia se niega con los motivos de no tener el sustento económico y por el alto índice de riesgos debido a su edad de 89 años, por lo tanto el paciente se encuentra en reposo en la cama de su casa, se mantiene la fractura con tablillas para evitar el continuo movimiento.


Con lo anteriormente mencionado es un paciente que lleva un control y registro diario dentro del área ambulatoria ya que al no tener permitido la autorización por parte de sus familiares, se busca promover la mejor atención desde su hogar, por lo mismo se le administran medicamentos analgésicos, hidratación y vitaminas además de orientar siempre a los familiares con los cambios posturales que se deben realizar para evitar úlceras por presión.

At the time of interviewing his family and advising on the fact that the patient should go to a medical center to be hospitalized since he requires surgical intervention, the family refuses with the reasons of not having financial support and due to the high risk index due to his age 89 years, therefore the patient is at rest in bed at home, the fracture is maintained with splints to avoid continuous movement.


With the aforementioned, he is a patient who keeps a daily control and record within the outpatient area since, since he is not allowed authorization by his relatives, he seeks to promote the best care from his home, for the same reason he is administered analgesic medications , hydration and vitamins in addition to always guiding family members with the postural changes that must be made to avoid pressure ulcers.

Nuestro equipo de salud para la visita domiciliaria se conformaba de 3 enfermeras, 1 médico comunitario y 1 servidor social.

Our health team for the home visit consisted of 3 nurses, 1 community doctor, and 1 social worker.
