ALL surgery carries a risk of death. People seem to forget that. Sometimes the anesthesia kills people. Sometimes they get infections that kill them. The WORST adverse effect is the failure to develop a healthy sense of self, and choosing instead to seek some unattainable "perfect" look and to have medical professionals essentially agree... "You are too ugly, lets fix you." That is a TERRIBLE way to provide "health care."
